When you decide to use SQL Server Authentication, you must provide a SQL Server user id and password. These are only used to build up a connection string to establish a connection to the database. It is never persisted by the app.
Other than that, this application does not collect or transmit personally identifiable information about you. No personal information is used, stored, secured or disclosed by services this application works with.
Limited technical information is sent to one organization under the form of crash reports.
